6 Outcomes for Medicare Patients Enrolled in a Value-Based Palliative Care Program: agilon health study

  1. spent an average of five more days at home during their final months (133.5 versus 128.23)
  2. were almost two-thirds less likely to die in a hospital (9% versus 26%).
  3. experienced 39% fewer hospitalizations
  4. experienced a 48% reduction in intensive care unit days
  5. experienced a 21% reduction in emergency department visits
  6. total cost of care was 33% lower ($10,393 on average) from the time of enrollment until death

Notes: Final sample included 1,778 decedents, with 889 in both enrolled and matched cohorts, average age 83, who were Medicare beneficiaries, died 1/1/2021 -1/31/2023, were patients of 8 primary care practices that partner with agilon health, and enrolled in palliative care for at least 7 days. Each practice operates within a value-based, full-risk model.
