1. unhappy with their former insurer: 22%
2. unhappy with copays and other out-of-pocket costs: 15%
3. their preferred doctor was no longer in network: 12%
4. changes to prescription coverage: 9%
5. premiums increased: 9%
6. moved to a new area: 8%
