www.HealthSprocket.com (the Web Site) is sponsored by Managed Care On-Line, Inc. (MCOL). The Web Site is designed to serve as a community for health care professionals and others to post, view and discuss lists pertaining to health care. The MCOL HealthSprocket Privacy Policy explains how we may collect, use and aggregate personal information about you on the Web Site, and describes the limitations we have placed on use of your personal information, and the protections we have implemented regarding the privacy of your personal information.

Your Acceptance of These Terms
By using the Web Site, you signify your acceptance of all provisions of the MCOL HealthSprocket Privacy Policy and other HealthSprocket Web Site Terms of Use. If you do not agree to the terms of the MCOL HealthSprocket Privacy Policy or other Terms of Use, please do not use the Web Site and exit the Web Site immediately. Your IP Address
For each visitor to the Web Site, MCOL's servers automatically utilize a "hit" counter and record the IP address or domain name. We use your IP address to help diagnose problems with our server and to administer the Web Site. Your IP address and domain name are anonymous (they do not reveal your personal identity) and are not shared with any third party, except when you specifically elect to disclose your e-mail address in content you submit as discussed below.

Registration and Subscriptions
The Web Site provides you with the opportunity to register to participate in activities described in the Web Site Registration Form including receiving e-mail news and announcements from HealthSprocket. You may opt to register to be eligible to participate as described by providing and submitting your name, e-mail address and other applicable contact and related information in the Web Site Registration Form. You may opt out of receiving e-mail information from MCOL (see the "Opt-Out Procedures" section below.)

Limitation on use of collected information
MCOL may provide individual subscriber email addresses and or other contact information to entities with an ownership interest in MCOL or entities that MCOL also shares an ownership interest in. MCOL may also provide to Sponsoring Entities applicable individual Member or Subscriber email addresses and or other contact information that have registered for specific items sponsored by the Sponsoring Entity. MCOL will not sell, exchange or otherwise share e-mail addresses and other contact information subscribed from the Web Site to any other party or organization without the explicit permission of the user. MCOL reserves the right to transfer title of its subscription registration information including e-mail addresses to a third party at any time. 

Data Integrity and Purpose Limitation
Personal information you provide to us is used to process your submissions or subscription, manage your account, deliver the services you request (including communicating with you about your submissions or subscription), and to respond to your inquiries and contact you our content and services on the basis of your consent and may be used in a number of ways including subscriptions to HealthSprocket publications or announcements, including e-newsletters; and to access certain features or functions of our content. MCOL takes reasonable steps to ensure that the personal data that it processes is: (i) relevant for the purposes for which it is to be used, (ii) reliable for its intended use, and (iii) accurate, complete and current. MCOL uses reasonable technological and physical safeguards to protect your personal information from loss, misuse and unauthorized access, disclosure, alteration and destruction. MCOL follows generally accepted industry standards to protect the information submitted to us, both during transmission and once received. The MCOL Data Privacy Officer has primary responsibility to oversee MCOL processing of personal data in this regard. Submission of Lists, Comments, Votes and other Information(Content)
The Web Site permits you to submit Content for public display and votes for Compilation. MCOL will not publicly identify or share the content of votes you submit from the web site with any third party, except in the numerical results in aggregate form for all applicable votes submitted. MCOL does require submission of lists and certain other information to be from Registered users as described above, and all Registered User content submitted will identify the Registered User name, and any additional information authorized by the Registered User as submitted in the Registration Form. Links
The Web Site contains links to other sites. MCOL is not responsible for the privacy practices or the content of such web sites, including any sites that may indicate a special relationship or partnership with MCOL, such as co-branded pages and "powered by" relationships. MCOL does not disclose Contact or Payment Information to those responsible for the linked sites. The linked sites, however, may collect personal information from you when you link to their site. This collecting of information is not subject to MCOL's control. To ensure protection of your privacy, always review the privacy policy of the sites you visit by linking from our Web Site.

Notification of Changes
The MCOL HealthSprocket Privacy Policy may be revised over time. The most recent update date of this policy will always appear at the beginning of this policy. We do, however, recommend that you read the MCOL HealthSprocket Privacy Policy each time you use this web site.

Correcting, Updating or Deleting Information
MCOL also permits you to correct, update or delete personal Information that you have previously provided to us. To do so, contact MCOL indicating the contact information that you wish to be corrected, updated or deleted, and MCOL will process your request in accordance with the provisions set forth in this policy. You may also request MCOL to send you an electronic copy of your personal data for your review.

Opt-Out Procedures
You may always opt-out of receiving future e-mails from us. To remove your Contact Information from our database, and to cancel future communications, send an e-mail to info@healthsprocket.com specifically indicating if you request to unsubscribe from e-mail news and announcements. This Opt-Out will remove you from HealthSprocket distribution lists, but would not remove you from any other distribution lists unrelated to HealthSprocket that you subscribed to separately with MCOL through other MCOL web sites. However, MCOL also provides separate opt-outs at any time for other MCOL distribution lists.

Your Rights

You have the right to:

• Withdraw your consent to the processing of your personal information at any time without penalty;
• Access your personal information and have it corrected, amended or deleted;
• Receive a copy of your personal data and transit such to others;
• At any time, to object to our processing of your personal information which we will comply with unless we demonstrate a compelling legitimate grounds for processing such that overrides your rights; and
• If you believe your personal information is inaccurate, unlawful, no longer necessary for our business purposes, or if you object to our processing of your personal information, you also have the right to instruct us to restrict the processing of your data pending our investigation and/or verification of your claim.
• If you wish to raise a complaint and initiate an investigation on how we have handled your personal data, or request a copy of your personal information, please contact us. We reserve the right to impose a charge for any direct external costs we would incur in responding to your request.
