5 NP and PA services that an ACO may offer to ACO REACH beneficiaries starting in 2023

1. certifying a Beneficiary as terminally ill for hospice care
2. certifying a Beneficiary’s need for diabetic shoes
3. establishing, reviewing, and signing an individualized cardiac rehabilitation care plan
4. establishing a plan of care for home infusion therapy
5. referring a Beneficiary for medical nutrition therapy services

Notes: The nurse practitioners (NPs) and physician assistants (PAs) must meet CMS eligibility requirements. These services ordinarily would need to be provided by a physician.
