  1. group A streptococcal testing among children younger than 3 years with pharyngitis: 37.6%
  2. computed tomography scan for minor head injury: 17.7%
  3. bronchodilators for treatment of bronchiolitis: 16.0%
  4. chest radiography for bronchiolitis: 15.6%
  5. chest radiography for asthma: 15.4%
