MedPAC: 6 issues that need to be resolved if two-sided ACOs are going to be part of the Medicare program in the long term

1. Are hospitals viable participants in ACOs?
2. Should asymmetric models be continued?
3. How should benchmarks be set initially and rebased for subsequent agreement periods?
4. Should the 5 percent bonus for clinicians in advanced alternative payment models (A–APMs) be distributed differently to encourage A–APM participation?
5. What will be the relationship between specialists and two-sided ACOs?
6. Are two-sided ACOs a long-term option in the Medicare program?
