Notes: Rankings for fully insured plans based on a 1,000 point scale. The avg ranking per plan was 671. The 2011 JD Power U.S. Employer Health Insurance Plan Study is based on responses from 7,024 employers, with quotas to assure an adequate distribution of small, medium, and large companies. The study was fielded between March and April 2011.
