  1. Influenza vaccination: increase of 0.18 per 100 patients
  2. Pneumococcal vaccination: increase of 0.56 per 100 patients
  3. Clinical depression screening: increase of 0.46 per 100 patients
  4. Annual wellness visits: increase of 1.52 per 100 patients
  5. Colorectal cancer screening: decrease of 0.03 per 100 patients
