  1. Congestive heart failure; nonhypertensive: 7.3%
  2. Septicemia (except in labor): 5.1%
  3. Pneumonia (except that caused by tuberculosis or sexually transmitted disease): 4.8%
  4. Chronic obstructive pulmonary disease and bronchiectasis: 4.2%
  5. Cardiac dysrhythmias: 3.8%
  6. Urinary tract infections: 3.1%
  7. Acute and unspecified renal failure: 2.9%
  8. Acute myocardial infarction: 2.8%
  9. Complication of device; implant or graft: 2.6%
  10. Acute cerebrovascular disease: 2.5%

Notes: Percentage is of total Medicare readmissions in 2011.
