  1. All-cause hospital stays per 1000 beneficiaries: 299.7 with HRSNs vs 188.0 without HRSNs
  2. Avoidable hospital stays per 1000 beneficiaries: 47.7 with HRSNs vs 19.1 without HRSNs
  3. All-cause Emergency Department (ED) visits per 1000 beneficiaries: 483.4 with HRSNs vs 290.1 without HRSNs
  4. Avoidable ED visits per 1000 beneficiaries: 197.5 with HRSNs vs 107.1 without HRSNs
  5. 30-day readmissions per 1000 beneficiaries: 9.5% with HRSNs vs 19.2% without HRSNs
