- Act: Put care plans into action; optimize decision support tools to match all stakeholders at the point of care and the point of decision
- Measure: Use clear metrics to assess PHM activities and stakeholder engagement
- Refine: Modify PHM program, care plans and clinical guidance based on near real-time evidence to optimize resources and quality of care delivered
