- Overall, individuals who identified social service needs had 68% higher odds of hospital readmission within 30 days of discharge than those without social needs.
- Those with financial needs had 19% higher odds of readmission within 30 days than those without those social needs.
- Those with food needs had 32% higher odds of readmission within 30 days than those without those social needs.
- Those with housing needs had 31% higher odds of readmission within 30 days than those without those social needs.
- Those with transportation needs had 21% higher odds of readmission within 30 days than those without those social needs.
