- Relative merits and challenges of physician-led vs. hospital-led accountable care networks
- The expense of setting up the infrastructure necessary for accountable care, for which there may be no source of payment in the near term
- The degree to which the new system will alter (or depend on) patient behavior
- The importance of developing multipayer alignment
