Six insights about PCMH initiatives in state Medicaid programs from KFF survey

  1. PCMH initiatives operated in 30 states' Medicaid programs in FY 2017
  2. 12 states reported plans to adopt or expand PCMHs in FY 2018, including Alaska, Arkansas, Colorado, Delaware, Georgia, Illinois, Michigan, New Mexico, Ohio, Pennsylvania, Tennessee, Wyoming
  3. 3 states reported the restriction or elimination of a PCMH program: Massachusetts, Maine and South Carolina
  4. 21 states had at least one ACA Health Home initiative, building on the PCMH concept, in place in FY 2017
  5. 7 states also reported plans to adopt or expand Health Homes in FY 2018: Alaska, California, Illinois, New Mexico, New York, Vermont and West Virginia
  6. One state --Ohio--reported plans to end its Health Home program in January 2018 as part of a behavioral health redesign initiative
