2. the “Renovated”: Team-based care, expanded and innovative access, electronic changes
3. the “Hybrid”: Expanding on team-based care to allow for targeted and improved care for different patient populations within the practice
4. the “Integrated”: Integration within the practice, as well as integration in the larger community and medical network where the practice resides
