1. “First Floor Team” of health coaches maintains close/continuous contact with patients, offering tailored services that respect patient preferences and cultural beliefs
2. "Second Floor Team” of medical doctors and nurse practitioners provide clinical care & empower patients to take responsibility for health and communicate openly with them
3. A “Third Floor” is a referral stream to specialists and acute care settings, selected based on quality and cost-efficiency performance
