Susanne Madden: What your patient portal needs in order to meet the criteria for both the NCQA PCMH and meaningful use programs

1. patients need to have online access to their health information (within a defined period of time)
2. a “clinical summary” must be made available
3. there must be secure two-way communication between your patients and your office
4. patients must be able to request appointments, prescription refills, referrals, and test results
