- Index admission for HF, AMI, PN or certain types of surgery
- Recent admission(s)
- Frequent ED visits
- Takes six or more medications
- CHF, diabetes, COPD
- Depression, psychoses
- Cancer, renal or lung disease
- Alcohol or drug dependency
- Disabled
- Frail
- Signs of poor nutrition
- Discharged during a weekend or holiday
