  5. Alabama - 7.4%
  6. Mississippi - 7.4%
  7. Arkansas - 7.1%
  8. Michigan - 7.1%
  9. Indiana - 6.9%
  10. Tennessee - 6.9%

Notes: Weighted estimates, age-adjusted to the 2000 U.S. standard population; Respondents were asked, "Has a doctor, nurse, or other health professional ever told you that you had angina or coronary heart disease?" and "Has a doctor, nurse, or other health professional ever told you that you had a heart attack, also called a myocardial infarction?" Refused, don't know, and missing responses were excluded from analysis.
