Abrams, Hospitals and Health Networks Daily. They talk about the need to define the population, provide a continuum of care, obtain performance-based contracts, and use of data.
 
Organizations that act fast will better navigate changes in regards to reimbursement. Developing a cross-system population health approach is crucial when deciding on a target population allows an organization to succeed across multiple populations in the future. When laying the groundwork of population health approach to care, organizations must be able to be proactive and prevent chronic care to a population. This would be during live and between encounters in regards to the health care system. Organizations seeking population health need to create contractual agreements with payers that are designated on meeting financial and quality metric criteria for managing patient health in all areas. Providers are required to have relevant health status and utilization data for patients at their disposal.
